Development Lead & Delivery Lead - Transforming Care
Contract type: Permanent - Full Time

We have two new and exciting opportunities for two exceptional leaders to help us lead this work and help us achieve our goal in supporting people with multiple and complex needs to flourish in the community.
We are looking for:
A Development Lead to respond to the need for individually designed and funded support for people who are currently in hospital or at risk of being admitted. Working within a complex commissioning landscape, this role will ensure the development of great support offers for people which enable them to live good, meaningful lives within local communities.
A Delivery Lead to implement and lead the specialist support offers ensuring people receive the best support, enabling them to live a good life as defined by them.
