We will support people who are returning to more independent living in the community after hospital, prison or residential care; recovering and rebuilding confidence after a crisis or breakdown in community living. We will focus on practical living skills, enabling people to live a good life alongside continuing complex mental, physical and social needs. Our enhanced offer will enable reduced hospital admissions at time of crisis with intensive and specialist support transforming practice with psychological support, support with substance use, healthy living/physical health and family support. Learning Disability and Autism We will offer a diverse and increased range of service offers to people who have learning disabilities and autism. We will focus on accommodation based support through supported living - including for those with the highest needs – reflecting our commitment to choice, control and independence. Our expert residential care will support people with very complex health needs, including end of life care and dementia. A significant and increasing proportion of our work will be with young people [16+] with very complex needs and their families as they plan for adulthood. Our commitment to whole family support will be evident with particular emphasis on families using respite support. Our enhanced and specialist offer for people with very complex needs will include positive behavioural support, healthy living/physical health support, communication and family support. Strategic Goal 1 We will replicate and expand our approaches to provide local community based alternatives to high cost rehabilitation services/ assessment and treatment in-patient care. We will be a purposeful and expert strategic partner, adding significant value to commissioners and other providers in tackling complex funding, housing and service delivery challenges through alliances, partnerships and collaborations We will improve, develop and expand our specialist support for people with: Certitude 2021: Forward Strategy 17  profound and multiple learning disabilities, autism and challenging support needs  complex mental health support needs and associated physical and social support needs
